What to ask before an ambient AI pilot.
Look beyond note generation to correction burden, consent, integration, and actual time saved.
Measure the work that remains after the draft appears. Faster note generation is not the same as less total clinical work.
Evaluate the whole encounter
Use a defined encounter mix and an agreed review rubric. Check omissions, unsupported additions, attribution, medication details, and whether the assessment reflects the clinician's reasoning. Include accents, interpreters, interruptions, and multi-speaker encounters in your approved test design.
Questions to take into the conversation
- How much editing is required before signature?
- Does the draft add findings that were never discussed?
- Which encounter types should be excluded from the pilot?
Make the data path explicit
Ask where audio, transcripts, notes, and logs travel; how long each is retained; and which subprocessors receive them. HHS explains that covered entities using cloud services for ePHI need appropriate BAAs and their own risk analysis. A vendor's security badge is not a substitute for that work.

- What happens when a patient declines recording?
- Can the agreement restrict training and secondary use?
- How are deletions, access controls, and incidents handled?
Count correction and integration costs
Compare total documentation time and after-hours work with baseline. Record adoption and abandonment, not just logins. Include training, templates, integration, support, and the effort of reconciling errors in the evaluation budget. These are suggested pilot measures, not promises about any vendor.
Questions to take into the conversation
- Does the system write to the intended EHR location with clear attribution?
- Who signs and remains responsible for the note?
- What result would justify expansion, and what would stop it?
